SUD Case Manager

Qualifications:

  • Bachelor's Degree (preferred) or minimum High school diploma
  • 1 year of SUD or behavioral healthcare experience
  • Individuals with lived experience or PRSS certification welcomed

Responsibilities Include:

  • Identify referrals ensuring quick and thorough transitions in and out of care
  • Coordinate communication and engagement across programs and levels of care
  • Facilitate supportive counseling groups
  • Participate in treatment planning meetings
  • Submit timely and accurate service documentation
  • Complete data collection and reporting 
  • Work closely with medical and behavioral health professionals to administer care
  • Collect feedback and advocate for program enhancements
  • Promote recovery and provide support for individuals in our programs
